The Family Health Network (FHN) has been recognized by the Community Health Quality Recognition (CHQR) for noteworthy quality improvement achievements.
Currently, the Family Health Network has received three distinctions, Silver Health Center Quality Leader Badge, Patient Centered Medical Home recognition, and Advancing Health Information Technology for quality.
- Silver Health Center Quality Leader Badge: Recognizes health centers that achieve best overall clinical quality measure performance among all health centers in the top 11-20%.
- Patient Centered Medical Home recognition: Recognizes health centers who have implemented a model of care that puts patients at the forefront of care.
- Advancing Health Information Technology for Quality: Recognizes health centers that meet all criteria to optimize HIT services which advance telehealth, patient engagement, interoperability, and collection of social determinants of health data to increase access to care and advance quality of care.
